Background: Hand-assisted laparoscopic nephrectomy (HALN) is the procedure of choice for living kidney donors. Our center is a public hospital that serves low-income patients from all over the country. HALN has been the routine procedure for the past 5 years.
Objective: Demonstrate experience and safety of HALN at our center and compare results to those of open living-donor nephrectomy (OLDN).
Methods: Observational, analytical, retrospective study was performed, reviewing data of HALN and OLDN, from March 1st 2009 to March 1st 2016. Demographic characteristics, renal function (pre and post-donation MDRD and serum creatinine [SCr]), length of hospital stay (LOS), bleeding and complication rates were compared using parametric tests according to the variables.
Results: 179 living donor nephrectomies were performed at our center from March 1st, 2009 to March 1st, 2016. Hand-assisted laparoscopic nephrectomy was performed in 148 cases (82.7%).

There were no differences in complication rates 30 days after surgery. Only 3 (1.7%) conversions to open surgery were reported.
Discussion: HALN has replaced open nephrectomy as the procedure of choice for living kidney donors. It has proven to be a safe procedure and feasible to perform at institutions in developing countries that care for patients of low financial income. At our center statistical significant findings when compared with OLDN are less surgical bleeding and shorter LOS.
