Purpose: In recent rears, a pilot program of organ donation after cardiac death (DCD) was initiated in China from general citizens. We describe the first successful series of DCD pancreas transplants in China.

Methods: Sixteen cases of diabetic nephropathy patients received simultaneously pancreas and kidney transplantation, with donors from DCD. All receipts and donors have same blood type and received cross match tests.

Results: All cases of patients were in postoperative 4-6 hours awake, 8-14 hours removal of the ventilator; after 2 days from SICU to common ward began to normal rehabilitation therapy. Serum C-peptide and insulin levels achieved normal range in 1-2 weeks after transplant; blood glucose returned to normal level in 2-3 weeks; creatinine decreased to normal level in postoperative 2 weeks. One case of patients has duodenal intramural hematoma during operation, and the pancreatic graft was removed for safe consideration. Other patients had no serious surgical complications within 2 weeks after transplantation.

Conclusion: DCD donor can be safely used in simultaneously pancreas and kidney transplantation. Pancreas and kidney functions recovered well in the early stage after transplantation, especially for young donors.
