Background:Shortage of optimal pancreatic donors stimulates a search for options to expand the donor pool. Extreme age donors ( <13 yrs or > 50 yrs old) are underutilized for SPKT. The aim of this study is to compare the outcome of SPKT using pediatric donors (PD), standard donors (SD) & Old donors (OD).Methods:Between 1988-2013,we performed 33 SPKT using PD(age<13 ), 651 using SD (age13-50) & 47 using OD (age>50) .Those cases were retrospectively reviewed based on patient & graft survival,transplant organs function(eGFR,serum glucose&urine amylase) and early technical graft loss. Results1,3,5&10 yr patient ,kidney&pancreas graft survival is shown in Figure 1.eGFR was higher in PD group compared to SD group at 2&5 yrs post-transplant (P=0.007).eGFR in the OD group was significantly lower at 1,2,3,4 and 5 yrs post-transplant (P=0.018,0.001,0.003,0.029 and 0.002 respectively).Average serum glucose was significantly lower in PD group compared to SD group at 2 yrs (P=0.021)&4 yrs (P=0.009).comparison of Average serum glucose between OD group and other groups didn't show any statistical significance. No difference regarding urine amylase was found.Early kidney graft loss (first 3o days)was 0%,1.4% & 2.1% for PD,SD&OD groups respectively. Early pancreas graft loss was 3%,4.8% and 8.5% for PD,SD and OD groups respectively.Conclusion:Using PD for SPKT has comparable outcome to SD & is associated with better grafts function without increased risk of technical complications. Compared to other age groups older donors are associated with significantly lower eGFR but comparable pancreas function and patient & graft survival.
