*Purpose: This study is aimed to evaluate Mayo Adhesive Probability (MAP) score as a predictor of adherent perinephric fat (APF) by analyzing a cohort of Japanese healthy donor underwent laparoscopic donor nephrectomy (LDN) for renal transplantation.

*Methods: A total of 71 healthy Japanese patients underwent LDN were evaluated in this study. The presence of APF was determined by recognition of adhesions between the renal capsule and perinephric adipose tissue from the intraoperative findings. Patients backgrounds: age, gender, body mass index, smoking and visceral fat volume, MAP score obtained from preoperative CT imaging were evaluated according to the presence of APF. The predictors of APF were examined using logistic regression analyses.

*Results: APF was observed in 19 (27%) patients. We observed APF in 2.4% of patients with a MAP score of 0, 57.1% with a score of 1, 50.0% with a score of 2, 50.0% with a score of 3, 85.7% with a score of 4. In univariate analysis, male gender, smoking history, visceral fat volume and higher MAP score showed significant differences between APF and no APF. In multivariate analysis, only higher MAP score was the independent predictors of APF (OR, 6.08; 95% CI, 0.11-34.0; p=0.039).

*Conclusions: MAP score can predict APF even in healthy kidney.
