Analysis of Patient Survival Outcome between Waiting List Adult HUS Patients Who Received Kidney Transplants (KT) and Those Who Did Not

Purpose: Our study aimed to determine the difference in survival probability between HUS patients listed for transplant who received a KT and those who remained in the waiting list (WL).
Methods:We studied adult HUS patients on the KT WL between 1/1/99 and 12/31/2009. 2 groups were compared : Group 1 received KT (RKT), Group 2 remained in the waiting list (RWL). We estimated survival probability difference between groups. We analyzed factors impacting patient survival by multivariate time dependent cox model.
Results:
For the period studied, 382 HUS patients were placed on the WL.71 % RKT.The RKT group was on average 9 years younger (38 vs. 47 yr old), less obese (14 vs. 25%), more likely repeat KT (26 % vs 1 %), than the RWL group (table 1). We found no significant difference in survival probability between the RKT and RWL groups.Multivariate time dependent cox model showed that candidate age was associated with higher risk for non-survival (1.065, P<0.0001) while private insurance with decreased risk by 60% (HR 0.39, P 0.0118) (fig.1)
We also compared the survival probability of HUS patients remaining in the WL vs Non-HUS patients who had KT and found a significant survival benefit for the latter (HR 0.828, P<0.0001).

Conclusions: Our analysis of SRTR data for the period 1999 to 2009, showed no significant difference in survival probability between adult HUS patients placed on the WL who received a KT and those who did not.Younger age and private insurance coverage are associated with better survival outcome among HUS patients placed on the WL. Non-HUS WL patients who received KT had better survival than HUS patients who remained in the WL.
