Non-adherence to immunosuppressants (IS) has been reported to increase morbidity and mortality of transplant patients.

Our objective was to investigate adherence to IS in renal transplant patients during the 3 first years post-transplantation.

EPIGREN is a cohort of 370 renal transplant patients followed-up for up to 3 years post-transplantation. Clinical data, quality of life (QOL) estimated with the SF-36 and adherence to IS estimated with the four-item Morisky medication (MMS-4) scales, were collected. Adherence was considered good, medium or low for MMS-4=0, 1-2, 3-4 respectively. K-means for longitudinal data was used to identify clusters of patients according to adherence and QOL. Pearson chi-square test, one way Anova and t-test were used to study relationships between covariates.

No patient was considered low adherent. The proportion of medium-adherent patients increased from 7% at M1 to 18% at M36. Two clusters were identified: adherence over time was good in cluster A and medium in cluster B. The proportion of medium-adherent patients at M1 was higher in cluster B than in cluster A (66% vs. 12%, p<10-6). Patients of cluster B were significantly younger than patients of cluster A (44 vs. 52 year-old, p=0.007). Medium adherence at M1 was significantly associated with a low mental HQOL score at M1 (OR=0.96, IC95%=0.93-0.99, p=0.010).

Two profiles of adherence over the 3 first years post-transplantation were identified. Adherence and QOL at M1 should be systematically evaluated, and actions to improve adherence should be proposed to younger patients, and patients with medium adherence or low mental QOL at M1.
