*Purpose: Post-renal transplant patients present a unique set of complex medical concerns that are best served by qualified, transplant-trained healthcare professionals. Post-renal transplant patients often experience electrolyte disturbances, fluid imbalances, medication management concerns, and post-transplant education difficulties during the first 3 months post-transplant; these often require medical intervention. Our purpose was to determine whether building a designated surgical renal outpatient infusion (SROI) room would lead to decreased admission rates, decreased cost, and possibly improved patient and physician satisfaction.

*Methods: A single center retrospective chart review of post-transplant patients in the first three months after transplant during the years 2011 (pre SROI room) and 2012 (post SROI room) was completed. Readmission rates, total cost, and patient satisfaction comments were collected.

*Results: Results showed a decrease in readmission rates in the first three months post-transplant after opening the SROI room, 37.5% in 2011 vs. 9% in 2012. Average cost savings was determined to be greater than $12,000 when compared to one inpatient admission. Patient and provider satisfaction were also improved.

*Conclusions: The SROI room has provided a cost effective alternative to inpatient admissions that has yielded a significant decrease in readmission rates along with improved patient and physician satisfaction.
