Objective To investigate the outcomes of kidney transplantation from pediatric donors to adult recipients, and explore the risk factors for DGF and graft loss after transplantation.

Methods We retrospective collected data of 170 kidney transplants from pediatric donors to adult recipients in our center from August 2011 to May 2017. The recipients were followed up for at least 6 months. The incidences of delayed graft function(DGF), acute rejection, hematuria, proteinuria, graft loss and patient death were all recorded.

Results 42(24.7%) patients had DGF, and 18(10.6%) patients had acute rejection after kidney transplantation. 94(55.3%) patients had hematuria and 22(12.9%) patients had proteinuria three months after transplantation. 1 year serum creatinine level was 95.3±21.2[mu]mol/L. 1-, 3-, 5- year graft survival were 94.1%, 88.4% and 85.3%, respectively. No patient died during the follow up period. Logistic multivariate regression showed that donor age<2 years old, donor body weight<10 kg, and DCD donors were independent risk factors for DGF after kidney transplantation. Cox multivariate regression showed that donor age<2 years old, and donor body weight<10 kg were independent risk factors for graft loss.

Conclusion The outcomes of kidney transplantation from pediatric donors to adult recipients were good. However DGF and graft loss rate were higher when donor age was very young with low body weight.
