Self-Reported Post-Surgical Pain Following Living Kidney Donation

The Kidney Donor Outcomes Cohort (KDOC) is an NIH-funded observational cohort study evaluating outcomes in living kidney donors (LKDs) at 6 US centers. LKDs completed the McGill Pain Questionnaire (SF-MPQ) at POD 1, 3, and then weekly for 8 weeks. The SF-MPQ assesses 15 pain descriptors (none, mild, moderate or severe), overall pain experience (no pain, mild, discomforting, distressing, horrible or excruciating), and present pain intensity (PPI, 0 to 10). Of 193 LKDs, 108 (56%) completed all pain assessments and 169 (88%) completed >7 assessments. 1,702 (88%) pain assessments were completed. The most commonly endorsed pain intensity adjective was “discomforting.” At POD 1, 12 (7%) LKDs reported pain intensity as “excruciating” and this remained constant over time. At POD 1, median PPI was 4 (IQR: 3) and by POD 35 the median PPI was 0 (IQR: 1). 157 (81%) LKDs self-reported a zero PPI at least one follow-up and never reported pain at subsequent follow ups. Main outcome was the point at which LKD reported zero PPI and had no pain at subsequent follow ups. Those who never 'progressed' to no pain by their final assessment were censored as were those lost to f/u. Kaplan-Meier survival curve shows median time to no pain was near POD 35. Hazard of no pain was estimated with a marginal Cox regression model. After accounting for center-level intra-class correlation and baseline pain, female sex (aHR = 1.14; 95% CI: 1.02,1.29) was associated with higher hazard of no pain and having a baseline affective disorder (aHR = 0.73; 95% CI: 0.60,0.89) was associated with lower hazard of no pain. A small subset of LKDs report present pain 8 weeks post-donation. Further investigation is warranted to examine this subset of individuals.
