Background: 98% of people on the First Person Authorization (FPA) registry for organ donation enroll at a Department of Motor Vehicle (DMV) licensing office. Donor conversion rates in our Organ Procurement Organization (OPO) were 89% for registrants compared to 54% of non-registrants who ultimately became donors. Government agencies run most DMV offices, but in Missouri organizations can bid to run DMV offices.
Methods: The OPO in a metro area won the contract to run 3 DMV offices and implemented a donor-centric approach. Employees were trained to ask each customer to enroll in the FPA registry. Decoration with donor-education material, fair pay for employees and customer experience improvements were also part of the OPO intervention. Data about registry signups and renewals were collected before and during the OPO DMV contract. Interrupted time series analysis was performed to evaluate whether the OPO intervention improved rates of FPA registration.
Results: Baseline registry enrollment rates were 10-50%. In the first year of OPO intervention, enrollment rates increased by 28.5%, 174.8% and 18.6% dependent on initial DMV baseline performance. After intervention at each office, the office with the lowest baseline enrollment (C) significantly improved FPA registration rates immediately (p<0.001) and the percent increase over time was steady (p=0.182). The office with mid-range baseline registry consents (B) did not change at the beginning (p=0.553), but registry sign-up continued to significantly improve over time (p<0.001). The office with the highest pre-implementation FPA registration rate (A), was better after OPO intervention, but the improvement was not statistically significant (p=0.067 (initial); p=0.932 (over time)).
Conclusions: A donor-centric OPO-run DMV experience increases FPA registry sign-up and renewal especially at offices with low initial registration rates. Ways to implement similar donor-centric DMV experiences at offices not run by an OPO are being developed.
