CT Based Anthropometric Visceral Fat Measures Predict Complexity, Complications and Low Clearance in Living Donor Nephrectomy

Donor nephrectomy complexity, complications and residual renal function remain subject to large degrees of unexplained heterogeneity. This study assessed intrabdominal and perinephric fat content using 15 different CT based fat calculations (incl. abdominal visceral fat content and perinephric fat ) in 500 patients undergoing donor nephrectomy. Analysis of each fat measure was carried out with spearman correlations, ttest and ANOVA. Multivariate regression models of the predictors of a low post donation clearance state (GFR <45mls/min) and operating time were also developed. The mean age of the cohort was 44.1 yrs, 17% were obese and 51% female. Abdominal visceral fat content was an independent predictor of operating time (coefficient 0.36 p=0.001), was greater in those developing postoperative infection (1692cm2 v 1489cm2 p=0.04) and also predicted those who developed low post donation clearance at one year (OR 1.1 (1.01 – 1.3 p<0.001).

Greater perinephric fat content was associated with incisional hernia development (235cm2v163cm2 p=0.04). BMI measurements demonstrated no relationship to outcome. Visceral and perinephric fat measurements are superior predictors of short and long term outcomes following living donation. In marginal cases it may facilitate peri-operative decision making.
