The UNOS Operations and Safety Committee is charged with reviewing reported patient safety situations, identifying safety gaps, addressing high frequency and high impact events, and to explore ways to disseminate information to the transplant community regarding adverse events to heighten awareness of safety. Total of 166 cases reported, 24 cases that were later excluded when there was no issue identified. All cases that resulted in non-recovery were related to living donor transplants, 5/9 were donor related and 4/9 were recipient related. There were 6 reported cases where organs were discarded, these were found to be related to packaging/shipping (4/6), recovery (2/6), and communication issues (1/6). There were 6 reported cases where the transplant procedure was significantly delayed, these were found to be related to transportation (3/6) and packaging/shipping issues (2/6).
