Objective: Creatinine based eGFR equations may overestimate the true GFR in simulataneous pancreas-kidney (SPK) transplant patients, owing to the low lean body mass in these patients. Inulin or iothalamate clearance is considered to be gold standard method of measuring true GFR. We sought to compare creatinine based eGFR and mGFR by iothalamate in this special population.
Methods: Iothalamate clearances were measured using single bolus injection, and following plasma clearance in five SPK patients at different intervals after transplant. These results were compared to eGFR values obtained using three different commonly used equations, namely, Modified Diet in Renal Disease (MDRD), Cockroft-Gault (CG), and Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI). T testing was utilized for comparing GFR measurements.
Results: There were 12 mGFR values done in 5 SPK recipients (4 males and 1 female) with baseline characteristics as shown in table 1.

The differences between eGFR estimated by each of the three equations and the mGFR (table 2). While all of the results were significantly different from the mGFR, the MDRD-based eGFR values were the closest estimate.

Conclusion: All the creatinine-based eGFR equations overestimated the true GFR as measured by iothalamate clearance. However, MDRD equation was the nearest estimate.
