Cross-sectional and Serial Evolution of Quality of Life in Elderly Kidney Donors.

Little is known regarding long-term health-related quality of life (HRQOL) in elderly (those donating when ≥65 yrs) living kidney donors (LKDs). Using the SF-36, we compared HRQOL in elderly LKDs to younger LKDs (50-64 yrs) and to the general population using published scores from the National Health Measurement Survey (NHMS).
In total, 25 elderly LKDs completed the SF-36 at 11±7 yrs postdonation. The avg age at donation was 68±4 yrs, 65% female, 61% related to their recipient, s cr at donation 0.9±0.2 mg/dL. Additionally, 425 younger LKDs completed the SF-36 and, aside from age, were characteristically similar to the elderly LKDs at donation.
The physical component score (PCS) for elderly LKDs was 47.8±10.2; NHMS controls, 49.2±15.1 (p=0.65), Table 1. PCS scores were also similar between elderly (47.8±10.2) and younger (50.7±10.5) LKDs (p=0.20); however this was a 3-point clinically meaningful difference. Mental component scores (MCS) were similar between elderly LKDs (54.4±10.8) and NHMS controls (53.8±13.1); and also similar between elderly (54.4±10.8) and younger (55.3±8.6) LKDs. Of the elderly LKDs, 13 completed serial SF36 surveys an average of 5 yrs apart; PCS and MCS scores were similar across both time points. Of all LKDs ≥50 years at donation, 18% had a PCS<40 and 12% had a MCS<40 at one or more time points, indicating physical and mental impairment. Both an increasing age at donation and increasing yrs from donation were associated with a 10% increased risk of PCS <40. There were no risk factors associated with MCS<40.
Conclusion: Mental HRQOL in elderly LKDs is comparable to younger LKDs and the general population. Consistent with expected consequences of aging, elderly LKDs reported lower physical HRQOL, but their avg scores remained in the normative range for the general population.
