Introduction : In recent studies, immune sensitization was associated with increased mortality in wait-listed kidney transplant candidates. We investigated whether the risk of immune sensitization to cardiovascular disease were present after kidney transplantation.
Methods : The Korean Organ Transplantation Registry (KOTRY) dataset was used for analysis. KOTRY is a systematic prospective multicenter cohort dataset of solid organ transplantation, which launched in 2014. Based on pre-transplant cardiovascular risk (age, diabetes, dialysis duration before transplantation, donors with brain stroke, desensitization of any cause), the predictors to acute cardiovascular disease (CVD) (within 6 months after transplantation) were assessed by multi-variable logistic regression to find odds ratios (OR).
Results : Total 1,460 kidney transplant recipients were analysed. Mean age at the transplant was 47.9±11.4 yrs old. Deceased donor kidney transplantation were 591 (40%). Diabetes mellitus were prevalent in 379 (26%) recipients. Desensitization was applied to 318 patients, mainly to desensitize anti-ABO or anti-HLA antibodies. Among living donor transplantation recipients, age (OR 1.09, 95% C.I. 1.04 – 1.15, p<0.001) and diabetes (OR 2.60, 95% C.I. 1.14 – 5.96, p=0.024) showed increased risk of acute CVD. Among deceased donor transplantation recipients, desensitization (OR 5.04, 95% C.I. 1.20 – 21.24, p=0.027) and diabetes (OR 4.10, 95% C.I. 1.79 – 9.38, p=0.001) showed increased risk of acute CVD.
Conclusion : The data offers evidence that increased cardiovascular risk in immune-sensitized patients are still present in early post-transplantation period, which needs further mechanism study.
