Background: The use of kidneys from Non-directed live donors (NDLD) among transplant centers in the US is unknown and the recipient selection remains varied. We explored trends in NDLD demographics and their recipients compared to directed live donors (DLD) and wait list candidates (WL).
Methods: SRTR data was used to explore demographics of NDLD and DLD from 2000-2013.
Results: The number of NDLDs has increased per year from 20 in 2000 to 154 in 2013. Compared with DLDs, a higher proportion of NDLDs were Caucasian (92.9% vs. 69.5% p<0.001), male (45.3% vs. 40.1% p<0.001), and educated with a college degree (67.6% vs. 49% p<0.001).

Conclusion:NDLD tend to be Caucasian, male, and have a higher education level. Recipients of NDLD kidneys have been on dialysis and waitlist for a longer time than DLD. NDLD has disproportionately benefited Caucasian patients.
