OPTN/UNOS began requiring 2-year living donor follow-up (LDF) in 2013. Current LDF, and changes associated with the new guidelines, have not been evaluated.
METHODS: Using SRTR data on 31,615 living kidney donors between 01/2010-06/2015, we compared proportions of complete/timely LDF form submissions pre- and post-implementation of LDF requirements, and used hierarchal logistic regression to assess center- and donor-level characteristics associated with complete/timely form submissions. We studied compliance as defined by OPTN/UNOS, i.e. meeting thresholds for the proportion of complete LDF clinical and laboratory data submitted in a timely manner (60 days before or after the expected visit date).
RESULTS: The post-implementation period was associated with a 41% increase in adjusted odds of complete/timely LDF clinical and laboratory data (aOR: 1.141.411.75). The proportion of donors with 2-year LDF clinical data increased from 49% pre-implementation to 64% post-implementation (p<0.001); similarly, 2-year LDF laboratory data increased from 37% to 56% (p<0.001). However, post-implementation, still only 43% (87/202) of centers were compliant with OPTN/UNOS-required LDF.
CONCLUSIONS: Fewer than half of transplant centers currently meet the national 6, 12, and 24 month LDF requirements. New practices and innovations are needed to better, more efficiently engage living kidney donors so that this unfunded OPTN/UNOS mandate can be met.
