Background. On July 3, 2014, the Organ Procurement and Transplantation Network/United Network for Organ Sharing (OPTN/UNOS) was charged with the oversight of Vascularized Composite Allograft (VCA) procurement and transplantation in the U.S. Prior to July 2014, there had been 28 VCA transplants at 11 centers. Since July 3, 2014, transplant centers have to submit a letter of intent and be approved to perform VCA transplants. The current abstract describes the latest VCA data in the U.S.
Results. As of November 27, 2015, there were 52 VCA programs approved in the U.S. representing 24 unique transplant centers. There were 17 upper limb, 14 head and neck, 12 abdominal wall, 1 lower limb, and 8 genitourinary organs (including 2 penile and 3 uterine).
The table below shows the number of candidates added to the waiting list and the number of transplants during 7/3/14-11/27/15, as well as the number of candidates still waiting as of 11/27/15. Twenty candidates have been added to the VCA waiting list at 9 different transplant centers. Nine VCA transplants have been performed at 8 centers (4 upper limb, 3 craniofacial, 1 scalp, and 1 abdominal wall). All 9 recipients were male and 6 were White. Four recipients were ages 35-44 yrs, 2 were 18-34 yrs, 2 were 45+ yrs, and 1 was <18 yrs. Three candidates were removed for non-transplant reason and 8 were still waiting for upper limb at 4 centers as of 11/27/15.

Of the 8 transplanted VCA donors, 2 were <18 yrs, 4 were 18-34 yrs, and 2 were 35-44 yrs. All of the donors were male and 6 were White. All 8 VCA donors donated at least one non-VCA organ resulting in 35 non-VCA transplants (6 heart, 2 intestine, 13 kidney, 1 kidney-pancreas, 8 liver, 4 lung, and 1 pancreas).
Conclusions. At present, VCA is still in its infancy and involving only a small number of patients. Data on post-transplant outcomes are being collected to further the understanding of this complex and evolving field. More detailed data will be important for policy refinement and ensuring patient safety.
