Predictors of Urologic Complications After Pediatric En-Bloc Donor Kidney Transplantation.

Pediatric en-bloc kidney transplant (PEBKT) has expanded the donor pool. Association of donor characteristics and urologic complications is not well described. The objective of this study is to determine donor characteristics associated with urologic complications in PEBKT.
This is a historical cohort study at a university hospital from 2003 to 2014 analyzing incidence of urologic complications associated with these donor characteristics: donation after cardiac death (DCD), donation after brain death (DBD), weight, warm ischemia time (WIT), and cold ischemia time (CIT). Urologic complications combined urine leak and ureteral stenosis.
Overall incidence of urologic complications was 21 (9.7%) of 217 included PEBKT. Donor weight range was 1.9 – 22 kg with incidence of urologic complications 17% in < 3 kg, 18% in 3-5 kg, 14% in 5-7 kg, 0% in 7-9 kg, and 6.9% > 9 kg (p = 0.032 Chi-squared test). Incidence of urologic complications was 12% in DCD and 8.5% in DBD (p = 0.4, Chi-squared test). No correlation with WIT or CIT based on logistic regression analysis.
In conclusion, size of the donor correlated with the incidence of urologic complications in PEBKT. Characteristics associated with organ preservation did not have a significant effect.
